Job Description: Part-time LPN {Wednesday - Friday 3pm-11pm} 24 HOURS/WEEK
Position Summary:
The primary role of a Licensed Practical Nurse (LPN) is to provide comprehensive oversight, coordination, and hands-on care in addressing all health-related matters affecting program participants. This includes both direct and indirect health concerns, ensuring the physical and emotional well-being of individuals through proactive assessment, timely intervention, and ongoing collaboration with interdisciplinary team members, families, and external healthcare providers. The LPN plays a critical role in maintaining a safe, responsive, and supportive environment that promotes optimal health outcomes for all individuals involved in the program.
Major Responsibilities:
- Maintains nursing documentation and medication records.
- Assists residents in learning about their medications; assures that medications are taken as physician ordered and recorded in electronic MAR.
- Provides nursing care including, but not limited to, wound care, trach care, diabetic counseling, etc
- Assesses residents’ health and emotional needs.
- Communicates with other members of the nursing and program staff; communicates serious issues to Nurse Manager and Program Director.
- Maintains confidentiality, including keeping voice low and not talking about residents in public areas
- Other duties as assigned.
Job Qualifications:
- Current PA RN/LPN licensure.
- Current CPR/First Aid certification.
- Valid Driver’s License.
- Act 33/34 clearances.
- FBI clearance as needed.
- All trainings required by licensure.
- Must complete a Physical, TB test, and Hep B titer.

Patient Care: LPN’s assess patient health, and implement nursing interventions to ensure optimal patient outcomes. Medication Administration: Administer medications and monitor patients for side effects and reactions, ensuring adherence to prescribed treatment plans. Collaboration: Work closely with physicians and other healthcare professionals to coordinate patient care and communicate updates regarding patient conditions. Documentation: Maintain accurate and complete electronic patient records, documenting vital signs, symptoms, and changes in patient conditions. Patient Education: Educate patients about health conditions, treatment options, and self-care strategies to promote recovery and wellness through individual and group sessions.
Proficient use of computers, mobile devices, and agency software. Business use of company equipment including vehicle.
Medium: exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds constantly to move objects.
