COMMUNITY INCLUSION SPECIALIST

The Community Inclusion Specialist (CIS) provides assistance to adults with developmental disabilities. The CIS creates and facilitates daily outings designed to promote community integration, build cultural identity, and connect participants with relevant opportunities to help meet their larger goals within the community. Participants may spend time in the community engaging in anything from cultural activities, museum visits, life skills-based excursions, volunteer opportunities, parks and more. The CIS may accompany up to three people at a time in the community, using public transportation or in your own personal vehicle.
This is a 34 hour position and it is considered full time, so you can choose from a great array of benefits and earn paid time off!
The shifts: Monday and Tuesday 8am-4pm, Wednesday 8am-2pm, Thursday 8am-12pm, Friday 8am-4pm.
Essential Duties and Functions:
- Supervise and oversee participants in the community daily to achieve their personal goals and take part in activities and/or events.
- Meet the basic needs of our participants while in the community. This may include assistance with other Activities of Daily Living (ADLs) as required by the participant.
- Implement IP’s (Individual Plans) and behavior plans, as needed.
- Maintain sufficient knowledge of participants’ IPs and personal needs required to work with them in a meaningful and productive way.
- Maintain tracking of receipts and expenditures for reconciliation, assist will bill paying and budgeting.
- Complete all required paperwork, including but not limited to case notes at the end of the visit.
- Drive personal vehicles or use public transportation to assist individuals in accessing the community.
- Mileage is reimbursable.
- Assist families with obtaining/maintaining basic entitlements and benefits as necessary.
- Attends all internal and external meetings on the treatment planning for each client on assigned caseload.
- Attend basic first aid, when needed.
- Assist with and/or provide and meal preparation. light housekeeping.
- Document supports given in the electronic visit verification system.
- Maintain annual PMT certification.
Qualifications:
- High School Diploma, Bachelor’s degree preferred.
- Three years’ experience working with people with developmental disabilities.
- Basic Computer skills using Microsoft Products.
- Valid driver’s license and clean driving record.
- Proof of active vehicle insurance.
This position requires medium lifting requirements, exerting up to 50 pounds of force occasionally.
