Case Manager – Homeless Services

Program Overview:
RHD Montgomery County Homeless Services offers a safe place to stay and assistance for individuals experiencing homelessness, helping them transition towards independent living. This is a new site located in Lansdale, PA, and the program specifically caters to single adults without housing. Clients have access to dedicated caseworkers who help them find permanent housing and apply for necessary benefits. Additionally, we provide practical support such as laundry facilities, transportation, meals, clothing, and essential life skills training.
This program provides a safe environment and comprehensive support services to help individuals transition to stable, independent living. Services include case management, housing support, life skills training, and access to essential resources.
Position Overview:
The Case Manager plays a critical role in supporting participants at Genny’s Place by coordinating services, connecting individuals to community resources, and helping them achieve their recovery and housing goals.
This role requires a compassionate, proactive professional who can advocate for participants, manage multiple responsibilities, and work collaboratively within a multidisciplinary team.
Key Responsibilities:
Team-Based Planning:
- Collaborate with a multidisciplinary team to develop and implement person-centered recovery plans.
- Partner with participants, clinical staff, and community stakeholders to support goal achievement.
- Build and maintain relationships with community agencies and service providers.
Direct Client Support:
- Provide community-based case management services focused on housing stability, recovery, and independence.
- Assist participants with accessing benefits, housing, and community resources.
- Transport participants to appointments and support engagement in services.
Administrative & Documentation:
- Complete assessments, service plans, and required documentation in compliance with organizational and regulatory standards.
- Maintain accurate and timely records in accordance with program and funder requirements.
- Identify service gaps and recommend improvements to enhance program effectiveness.
Qualifications:
- Bachelor’s degree in Human Services or related field preferred (or equivalent experience).
- Minimum of 2 years of experience supporting individuals in behavioral health, substance use, or IDD services.
- Case management experience preferred.
- Valid driver’s license and reliable transportation required.
- Intermediate computer skills and ability to use electronic systems.
