Case Manager
RHD | Behavioral Health Program

RHD is seeking a compassionate, organized, and motivated Case Manager to support individuals in achieving their recovery goals through person-centered, strengths-based services.
In this role, you will work closely with participants, clinical staff, and community partners to ensure individuals receive the services and support they need to thrive.
What You’ll Do:
Person-Centered Planning & Advocacy:
- Serve as an advocate to ensure participants receive needed services and supports.
- Collaborate with a multidisciplinary team to provide holistic, values-based care.
- Assist in developing and reviewing individualized recovery plans.
- Identify service gaps and recommend strategies to improve participant outcomes.
- Build relationships with community agencies to connect participants to resources.
Direct Community-Based Support:
- Provide community-based services focused on education, resource connection, and goal achievement.
- Support participants in achieving recovery plan objectives.
- Transport participants to appointments as needed within the service area.
Documentation & Compliance:
- Complete assessments and maintain accurate documentation in compliance with RHD, funders, and regulatory bodies.
- Ensure adherence to policies, procedures, and professional standards.
- Perform additional duties as assigned.

Qualifications:
Education & Experience:
- Bachelor’s degree in Human Services or related field preferred (or equivalent experience).
- Minimum 2 years of experience supporting individuals in behavioral health, addiction recovery, or IDD services.
- At least 1 year of case management experience preferred.
- Intermediate computer skills.
- Valid driver’s license and reliable transportation.
